Your initial appointment will involve a consultation with Dr Chahoud explaining your diagnosis and treatment options after a thorough examination and medical history have been undertaken. It is also an opportunity for you to ask any questions or raise any concerns that you may have.
Following consultation, our team will work in partnership with you to obtain the best possible results. One of our friendly staff will issue you with all of the necessary information involving future hospital bookings, written quotes and pre and post surgical instructions.
Please assist us by providing the following information at the time of your consultation:
- Referral
- OPG x-ray (plus any others you may have been given)
- Medicare Card
- Private Health Insurance details
- Veteran affairs card (if applicable)
- List of any medications you are currently taking
- Form of payment (EFT, MasterCard or Visa accepted)
- Any medical reports relevant to your condition
- Diary, rosters, work commitments
